The DeKalb Board of Commissioners will hear these applications for Special Land Use Permits.

Address: 5565 Mill Trace Drive

Application of Mill Glen Swim Club for a Special Land Use Permit to allow a Neighborhood Recreation Club on property located in the R-100 zoning district located on the southwest side of Mill Trace Drive, approximately 250 feet southeasterly, easterly and northeasterly of Mill Trace Court. The property has frontage of 24.65 feet and contains 3.43 acres. This is an existing recreation club. The applicant is proposing to renovate and expand the existing clubhouse, expand the pool deck and construct a new practice tennis court.

Address: 1259 Parkside Drive

Application of Parkside Brookhaven, LLC for a Special Land Use Permit to allow a building in excess of 2 stories in height. Subject property is located within the C-1 zoning district, on the west side of Apple Valley Road and southwest side of former Hill Crest Road. The property has frontage of 61 feet on Apple Valley Road and contains .73 acres.

Address: 5526 Hillandale Road

Application of Salem Baptist Church of Atlanta, Inc. for a Special Land Use Permit to allow a building in excess of two stories\25 feet in height. Subject property is located within the C-1 zoning district, on the northeastern side of Hillandale Road 866 feet southerly along the variable right of way of Hillandale Road from the intersection of Panola Road. The property has frontage of 2061 feet on Hillandale Road and contains 33.394 acres. The applicant is proposing to build a parking ramp with 6-stories above ground.

Address: 4800 Flat Shoals Parkway

Application of American Tower for a Special Land Use Permit to allow a Telecommunications Tower (Multiple wireless-use communications facility) on property located in the NS and C-1 zoning districts. Property is located on the northeast side of Flat Shoals Parkway 200 feet westerly of Wesley Chapel Road and 225 feet northerly from Flat Shoals Parkway on the west side of Wesley Chapel Road. Property has 578 feet of frontage along Flat Shoals Parkway and 605 feet of frontage along Wesley Chapel Road and contains 14.33 acres. The applicant seeks to construct a 150-foot monopole and a 12'x28' equipment shelter enclosed within a 60-foot by 60-foot fenced area.

Address: 3074 Rainbow Drive

Application of Harden Leroy Lark for a Special Land Use Permit to allow a Place of Worship in the R-75 zoning district. Property is located on the north side of Rainbow Drive, 2188 feet easterly of Candler Road. Property has 203 feet of frontage on Rainbow Drive and contains 3.074 acres. The Place of Worship is an existing use. The applicant seeks to construct a new 5000 square foot (300 seat) sanctuary building, additional parking, and use existing building as a Fellowship Hall.

Application of Eileen C. Weber for a Special Land Use Permit to allow a Home Occupation (Hair Salon) with customer contact in the R-100 zoning district. Property is located on the northwest side of Springfield Court and approximately 135 feet southwesterly of Springfield Drive. Property has frontage of 120 feet on Springfield Court and contains 18,700 square feet. Applicant seeks to operate a hair salon with customer contact in her home. . q 3 Ac.

Address: 2828 Wesley Chapel Road

Application of Wesley Chapel United Methodist Church for a Special Land Use Permit to allow a Place of Worship in the R-100 zoning district. Property is located on the southeast side of Wesley Chapel Road and northwest side of former Old Wesley Chapel Road. Property has 1143 feet of frontage on Wesley Chapel Road and contains 11 acres. This is an existing Place of Worship. Applicant seeks to expand parking lot.

Address: 7232 Browns Mill Road

Application of Philadelphia Baptist Chruch for a Special Land Use Permit to allow a Place of Worship in the R-85 zoning district. Property is located on southwest side of Browns Mill Road approximately 1100 feet southeasterly of Browns Mill Drive. Property has 1264 feet of frontage on Browns Mill Road and contains 15 acres. Applicant seeks to construct an 18,000 square foot worship building (460 seats), 15,400 square foot building (gym/education), and a 124-space parking lot.

Address: 5665 Peachtree Dunwoody Road

Application of Saint Joseph's Hospital of Atlanta, Inc. for a Special Land Use Permit to allow a building exceeding 5 stories in height in the OI zoning district. Property is located on the north side of Old Johnson Ferry Road approximately 542 feet northwesterly of Byrnwyck Place. Property has frontage of 131 feet on Old Johnson Ferry Road and contains 18.860 acres. Applicant is seeking to construct a 6-story parking deck.

10. SLUP-00030

Commission Districts: 5&7

Address: 3800 Linecrest Road

Application of Anointed Word Christian Ministries International Inc for a Special Land Use Permit to allow a Place of Worship m the R-100 zoning district. Property is located on the northwest side of Linecrest Road 325 feet northeasterly from Catalpa Lane. Property has 375 feet of frontage on Linecrest Road and contains 6 478 acres. Property is currently used as a single- family residence. Applicant is seeking to construct a new church building, which will lions, their worship center, offices, fellowship hall, Sunday school rooms and church school.
